  1. iPad版Trail making testを用いた高次脳機能スクリーニングテストへの応用可能性, 濱 聖司, 柳川 亜紀子, 第８回　日本リハビリテーション医学会秋季学術集会, 2024年11月03日, 通常, 日本語, 日本リハビリテーション医学会, 岡山, 【目的】iPadを用いてデジタル化したTrail making test (iTMT)が、簡易的な認知機能スクリーニングテストに応用できるかどうかについて、Mini-mental state examination (MMSE)の推定を通して検討した。【方法】リハビリテーション目的で入院している患者42人（30人は脳卒中)を対象に、iTMTを5回(最初の3回:iTMT-A、4回目:TMT-Aの紙版、5回目:iTMT-Aの逆バージョン)、MMSEを1回実施した。各iTMT-Aの遂行時間は、移動時間と滞留時間に分けた。繰り返し効果は５回の遂行時間を線形混合モデルで解析。MMSEスコアの推定は３回のiTMT-AとiTMT-Bの遂行時間、あるいは3回のiTMT-Aの滞留時間、移動時間とiTMT-B可否を変数として偏最小二乗(PLS) 回帰で行った。【結果】線形混合モデルでは、3番目と4番目のiTMT-Aの遂行時間が最初のiTMT-Aよりも早いことから、学習効果が示唆された。PLS回帰では、MMSEスコアが低い被験者に対しても、iTMT-Aの繰り返しから抽出した滞留時間と移動時間、TMT-Bの可否を用いることで、iTMT-A & Bの遂行時間のみを使う解析（R2=0.45）よりもMMSEスコアを推定するための決定係数を高めた（R2=0.72）。これらの知見は、iTMT-Aの滞留時間が認知機能の推定に重要であることを示唆する。iTMT-Aは、時間的・空間的に多くの因子を抽出し、繰り返し試行の学習効果を取り入れることで、少ない試技で多くの脳機能をスクリーニングできる可能性が示せた。
