研究：主として歯科パノラマエックス線画像からの骨粗鬆症スクリーニングに関する研究を行っております。 教育：歯科領域の画像診断学を担当しております。 診療：顎顔面領域を画像診断に専ら従事しています。

  9. 顎下部へ向け大きく屈曲した走行を認めた総頚動脈走行異常の1例, 中元　崇、末井　良和、福谷　多恵、小西　勝、長﨑　信一、藤田　實、 柿本　直也, 日本歯科放射線学会　第36回関西・九州合同地方会, 2016年12月10日, 通常, 日本語, 日本歯科放射線学会, 福岡市, 我々は、顎下部への総頚動脈の走行異常が認められた症例を経験したため、これを報告する。 患者：86歳、女性 主訴：顎下部の腫脹 現病歴：平成28年4月初旬、近医歯科医院での歯科治療中、右側顎下部に腫脹を自覚したため、リンパ節の腫脹を疑い、紹介来院した。 既往歴：高血圧 画像所見：造影CT像にて右側総頸動脈が大きく前方に向け屈曲し、顎下部皮下をループ状に走行している像が認められた。超音波検査においても、同部を走行する動脈が確認された。いずれの画像検査においてもリンパ節は確認できなかった。これらの画像所見から総頚動脈の走行異常であると診断された。 過去の文献では、高血圧を有する高齢女性の右側総頚動脈に多くの報告がある。高血圧による大動脈の圧負担による影響ではないかと考察している報告がみられた。本症例も高齢女性であり、走行異常は右側に見られた。一般的には加療は不要だが、血栓形成など、一過性脳虚血発作の原因になる可能性があるため、注意が必要であると思われた。
